The monthly Intentional Aesthetics and Wellness (IAW) Subscription Program is designed to assist our patients wishing to be intentional about achieving their aesthetic and wellness goals. The program provides predictable budgeting, offers exclusive subscriber-only pricing, and encourages long-term treatment planning/preventative care.

The IAW Subscription allows subscribers to deposit a fixed monthly amount into a personal "Intentional Bank" account. These funds may be used towards ANY aesthetics and wellness services, as well as retail products, offered within the practice. Subscribers will commit to a minimum monthly amount of $139, however can choose a greater amount of $199 or $299 per month if desired. One hundred percent of the monthly payment is credited to the subscriber's Intentional Bank balance.

Monthly payments are automatically charged to the card on file. Payments will be processed on the same day every month that the subscription was initiated. An annual processing fee of $48 will be charged separately and on the day of initiation into the program. Subscribers are authorized recurring charges until cancellation. Declined payments must be resolved within seven days to maintain benefits.

Subscription balances never expire; however, the funds are non-transferable and must be used only by the subscriber enrolled in the program.

The initial subscription period shall be for a period of 12 months. A subscriber shall not be entitled to cancel his/her subscription during the initial period. After the initial period, the subscription shall continue on a month-to-month basis, and the subscriber may cancel the subscription at any time by giving a 30-day written notice. Any unused amount in the subscriber's account, upon cancellation, shall remain in the account and can be used at any time without expiration. However, a monetary refund shall not be granted to the subscriber upon cancellation.

There may be times when extenuating circumstances, such as extended illness, military service, or temporary relocation, cause a subscriber to need to suspend their subscription. A subscriber may suspend their subscription for up to a total of three months during the initial period by providing written notice 30 days prior to the beginning of the date the suspension is to begin. This will extend the contract period by the number of months the subscription is suspended. IAW reserves the right to cancel any subscription at any time.

A valid ID and credit card are required to enroll in the IAW Monthly Subscription program. The subscriber agrees to provide IAW with updated information if the address or payment details should change. If IAW is unable to process the automatic monthly dues, the subscription will be suspended pending payment in full.
